New Patient Registration

Why should you Register with Your Local GP

We are a non-discriminatory and non-judgmental practice. We will, on the basis of eligibility, accept patients without considering their race, gender, social class, age or religion.

If you wish to register with the practice you should collect registration forms from either of the surgeries.

You will be asked to complete a new patient questionnaire which gives us an indication of your past medical history and allows us to provide appropriate care while we wait for your medical records to be transferred from your previous practice. When you bring your registration form to the surgery our administration team will ask you to supply proof of your address, please bring this with you, it will take 72 hours to register a new patient.

Change of Address

If you move home, please let us know your new address and telephone number as soon as possible. You may miss important letters or appointments if you do not help to keep our records up to date. Remember hospital appointments will be sent to the address that we have on our records so it is very important that we know where you live.

Leaving the Area

If your change of address shows that you are leaving our practice area we will write to you confirming that you should make arrangements to register with a practice closer to your new address.
